Winter Court Sports Can Equal Foot and Ankle Pain!

Ankle pain messing with your game? With the pounding and movements that feet, ankles and the Achilles and other tendons around the ankle endure, court sports such as basketball, racquetball and tennis present unique problems for athletes, whether they`re NBA types or weekend warriors. But preparation and common sense go a long way toward prevention.

Hard surfaces and sports calling for fast, lateral movements are two characteristics of indoor sports that can contribute to injuries. One of the most frequently encountered court injuries is the common ankle sprain, due to the lateral aspect of those sports and, especially in basketball, the potential for stepping on a competitor`s foot.

Other court injuries are more too much shock-related stress, such as stress fractures in the foot or tibia. Stress fractures can be hard to detect and many times people with stress fractures will continue to play and delay the correct diagnosis.

If the stress fracture is in the metatarsal bone, the most frequent site for stress fractures, there can be swelling in the forefoot. But stress fractures can occur in any of the 26 bones of the foot without many symptoms. Some are more common than others, but all of them can happen in court sports. In the stronger, more dense area of the mid-foot, stress fractures can take a long time to diagnose, and if not properly diagnosed, can lead to devastating consequences. Whether competing in court sports on an organized level or in everyday use, the foot should not hurt. If pain persists, consult a foot and ankle surgeon.

Another common injury site is the heel. Persistant heel pain can be a plantar fascia injury or a heel spur. The plantar fascia is a thin, very strong tissue band that supports the arch of the foot, stretching from the heel to the ball of the foot. As the arch falls, the band of tissue is under increasing tension and the weakest link is where the tissue attaches to the bottom of the heel. Over time and with repeated shocks, micro-events can occur and cause the fascia to tear from the heel, resulting in bone spurs.

Ninety-nine percent of the time, conservative care is effective in relieving painful heels. For the few who don`t respond, surgery may be considered. Foot and ankle surgeons frequently see injuries to the Achilles tendon and other tendons about the ankle that often could be prevented. Achilles and calf stretching can go a long way toward prevention. A ruptured Achilles tendon requires surgery and is very common in older athletes who aren`t flexible and don`t warm-up.

For a gentle stretching of the lower extremities and the Achilles tendon, lean against a table or wall, placing one foot behind the other. Point the toes slightly inward and roll the ankle toward the outside. Gently lean forward and feel the stretch high up into the calf muscle. Always roll the weight to the outside border of the foot and never stretch to the point of producing pain.

Additional tips include:
1. Buy new, quality athletic shoes and replace them frequently (at least every 6 months).
2. Have the proper footgear for the sport; running shoes are not what you shold be wearing for sports such as basketball due to their lack of lateral support.
3. Seek appropriate arch support; flat and high arches often call for custom-made orthotics in shoes.
4. Avoid too much, too hard, and too soon syndrome; start slowly to transition between activities or the stress will take you out of the game quickly.
5. In children, be aware of the potential for injuries of the growth plate in the heel. Persistent heel pain is more than simple overuse; kids should stop participation immediately and a diagnosis should be sought. Boys and girls often go from soccer season right into basketball and the collective shock from running and the pull of the Achilles can cause growth plate damage.

If you are doing lots of rehearsals for the Nutcracker or some other holiday season dance concert, you need to take special care of your joints, muscles, and feet. Wool legwarmers, snug sweaters and water-proof snow boots all contribute to a better holiday season if you are a ballet dancer.

Rehearsing classical ballet concerts takes a lot of organization. Most ballet dancers get good at this in their young years. If they haven`t caught on to the need for methodical self-preservation in the long process of rehearsing and staging a show, the experience may be uncomfortable to say the least.

Starting with extra ballet wear, packing some sweats to wear in long rehearsal days is a must. Sweaters, shawls and scarves, help a lot to keep cool drafts off neck and shoulder muscles.

Leg warmers save the day to help warm up and to stay warm while waiting for your turn on stage.

In long technical rehearsals, the wait can be exasperating while lighting cues are tested. Fatigue can be challenging when repeated run throughs are needed for the stage crew and stage manager, or for dancers who are thrown into the dance at the last minute because of another`s injury, or winter sickness.

Anything can happen! If your mother puts some vitamin pills at your plate, take them! Stay healthy and be smart!

Long hot baths with Epsom Salts followed by icing sore spots will help a lot. Even ice the not-so sore spots, the tired muscles and joints. Give them a holiday present.

Drink enough water – sip all day. Don`t fill up with those frankenstein neon so-called sports waters, or sodas. Green juices and real fruit drinks are so much better, but water is best. Fruits for a snack are great. And one of the best snacks is celery. Celery is full of the mineral salts that you lose in perspiration. Losing those mineral salts decreases your muscle strength and your thinking power! Just the opposite of what you need to dance your best all day long.

You must take care of minor aches and pains, to make sure that they remain minor. Nerves and adrenaline get you through a lot, but be careful not to get drained. Your muscles and nervous system need real fresh foods to be replenished with.

The repetitive motion of rehearsing can lead to joint pain, raw pointe shoe blisters and deep aches in your muscles. Don`t let these conditions dull the thrill of your Nutcracker ballet performances.

How Physiotherapists Treat Neck Pain from Cervical Disc Prolapse

A prolapsed disc or injury to the nerve exit foramen in the neck can give very severe neck and arm pain known as cervical radiculopathy. The sixth cervical nerve is affected in 25% of cases and the seventh in 60% of cases. Of all arm pains of neck origin, about a quarter are due to an acute disc prolapse. With age disc bulges, ligament and joint enlargement and bony osteophytes encroach on the space the nerve has to travel through and this is a more common cause of cervical radiculopathy in older persons. Neck pain from cervical disc prolapse is routinely assessed and treated by physiotherapists.

Risk factors for this type of neck pain and arm pain include smoking, lifting heavy weights regularly (e.g. 12kg, 25 pounds) and driving or operating vibrating equipment. Overall cervical radiculopathy is uncommon and much more so than lumbar disc syndromes such as sciatica. The discs between the vertebrae from C2 to C7 transmit loads down through the spine and dissipate some of the forces applied to it. At the side of the vertebrae are the nerve exits or foramina and the nerve takes up to a third of the exit space normally. Degenerative changes in any of the structures which surround and form the walls of the exit can compromise the exit channel itself and compress the nerve.

Cervical radiculopathy can occur for a variety of reasons while sometimes there is no obvious precipitating factor that can be found. A sudden extension, side bending and rotation, e.g. in sport or trauma, could close down the exit gap quickly and squeeze the nerve, injuring it. Sudden flexion or extension with a side bend away from the affected side could pull the nerve strongly, again injuring it and causing symptoms. Disc prolapses can occur with a sudden load on the neck in any position, and in older people sustained or repetitive extension and rotation may cause the osteophytes to contact the nerve, with similar results to the previous events but with a slower onset.

The physiotherapist will take a detailed history to establish the diagnosis is clearly that of cervical radiculopathy. The exact location and nature of the pain, numbness or weakness, aggravating or easing factors, mechanism of injury, previous occurrences, lower limb symptoms, bladder or bowel dysfunction and medical or other treatments are all noted.

People with root pain look tired due to poor sleep, don`t find anything funny and guard their arm in a protective posture against the abdomen or hold it out to the side with their hand on the back of their neck or the other side of the head. This may reduce the forces through the inflamed nerve root and so reduce pain.

On physiotherapy examination patients look tired as they have not slept and they lose their sense of humour. They may hold their arm in a relieving posture cradled across the body or with the elbow out to the side and the hand behind the neck or over the head towards the opposite ear. This may reduce the tension through the irritated or compressed nerve root, reducing the pain.

Initial management is to reduce the inflammation and pain and physiotherapists use cold therapy, anti-inflammatories, manual traction, mechanical traction and instruction to avoid activities or postures which are worsening. The physio reduces the forces applied to the nerve root area with a collar for day or night use to support the neck and limit the available neck movements and with careful manual tractioning to relax muscle spasm and allow the pain to settle.

Reducing the pain and inflammation is the first goal of treatment and the physiotherapist can employ analgesics such as NSAIDs, cryotherapy, mechanical or manual traction and avoidance of aggravating activities and postures. Limiting the forces transmitted through the nerve root is an overall goal of management, using a collar to reduce neck movement, a cervical pillow or collar at night and manual traction from the physio to distract the joints. After the acute phase has settled physiotherapy concentrates on regaining neck movement and muscle power, starting with isometric exercises and moving on to isotonic and exercises for multiple muscle groups. Long term adherence to a regime of aerobic exercise, muscle strengthening and stretching may be useful.
